If Survey123 has not yet been installed on the device, the field worker must first install Survey123. Assuming that Survey123 is installed, once a field user has been added as a member to a Trekker Survey project by the Lead and is ready to perform data collection for the first time on a project, the first thing to be done is to download the two Survey123 forms for the project (Building Inventory Form and Proposed District Form).

Note that the forms only need to be downloaded once (per device) at the beginning of a Trekker survey project.

It should be noted that although the Building Inventory Form and Proposed District Form are identical templates for all Trekker Survey projects, the system creates unique forms for each project to ensure that only those users who are members of the project team can download the forms and capture data for a given project.

To download the forms for a survey project, open Survey123 on the device and login. The My Surveys page is displayed as shown below illustrating all the forms that have already been downloaded to this device.

To download the forms for the new survey project, click the icon that appears as three horizontal lines in the upper right of the application. This will slide a panel to the left, offering several menu choices as shown below.

Click the Download Surveys option and a selection of survey forms for all the Trekker survey projects for which you are a member will be displayed. To search for the specific forms to be downloaded, enter the Trekker Survey project number (e.g., 19SR00035) in the search bar at the top of the application and press enter. This will filter the list to only those forms that contain the text entered. Two survey forms should be visible, one named “yySRxxxxx - Inventory form” and the other named “yySRxxxxx – Proposed District” (yySRxxxxx is your survey project number from CRIS) as shown in the image below.

Click the cloud icon on the right side of the Inventory Form to download. When it is complete the following message is displayed.

Click the Ok button to dismiss the dialog and click the cloud icon for the Proposed District form to download. Once both Survey123 forms are downloaded for the new project, the field work can commence.
